Owner Surrender Request

Please note that this form is for requests to surrender Dogs Only. We do not have foster homes for cats. If you are inquiring about surrendering a farm animal, please email us at info@secondchanceanimalsanctuary.ca

If we are able to intake your dog, you will be required to sign an owner surrender form, provide us with all previous vet records and provide an owner surrender donation (suggested donation is $200 but we understand everyone’s circumstances are different). The surrender donation is used for vet care for the dog.
